The cells should get a bit higher than that even if the battery is old. If the battery is really used then the resistance will be high and the cells may never reach 4.2v but they should be closer than what you posted.
Only you know the life history of the battery.
How long is the battery on the charger before it kicks off?
Does the display read "Full" ?
Is the charger set to balance mode, fast charge mode or charge mode?
