Ouch on the screws falling out... Sounds like the prop was out of balance.. Never have screws shake loose albeit I've recently changed prop balancers which made a significant difference on vibration problems. Dubro balancers = the shiz! Top Flite balancer = junk. Tried new shafts and new cones from hobby services and those were worse than the ones that came with the balancer.
