At our field....maybe like yours....it is rarely Pilot Error....it's always 'I had nut'n and she went in'......so that form would be very efficient - with only one box checked 99.9% of the time....... sort of like the 2013 Tax forms they are sending out:

List on line 1 the amount you made in 2013
List on line 2 the same amount you stated on line one as your tax for 2013!

no need for anything complex

After reading the entire report, I see why it takes a year or more to release an accident report!
