Default Shorted cells ??

Yesterday I was cycling some B&K 9.6V batteries for our high school. They were having problems. After form charging one, it was determined they are about 1000mAH batteries with 8 sub C cells. But the story is about one of them it measured 1.2 V….. Took it apart and found all but one shorted. ??? also the wall charger had a blown primary transformer. None of the cells were swollen or had any out gases signs. What do you think they did to this pack to short 7 cells?

Is this common with these multi packs ? If one cell shorted you would not know only approximately 12% reduction in power you would think it was just getting old but now the wall charger is over charging the other cells. Short time for serious problems.
Have any of you seem such a thing Rich?
