Jason Shulman's Crash at TOC

I was just watching Model Sport Magazine's coverage of the TOC in Vegas. I was surprised to see Jason's crash. Does anyone know what led to the crash? Just curious.

Pilot error - Jason forgot to switch to 3D rates before the blender.
