Anyone have any direction on how to install and use this tool?

You need to take everything above the swashplate off and disconnect the motor and slide the tool on the shaft then adjust the links to make the swashplate touch on all planes.

Yep, then you put it all back together and test fly it and change all the links again. [:D]
Thats why its only a hobby, you have all the time in the world to tinker around with it.

no, it's meant to set up the swashplate only. once you have the swashplate level at mid, top and bottom stick you take if off and put the head back on.

Yes, Druss is correct here. You want to get it level at the top, mid and bottom, not just in the middle, this is where the swash leveler really helps. You can set your travel of each servo to get a perfect movement throughout the range.
