Accident occurred : Monday, May 26, 2003 at ST. THOMAS, VI
Aircraft : Modeltech 'ME-109 .40'
Engine : YS .53
Pilot: "visioneer_one"
Injuries: Multiple, fatal.

This is preliminary information, subject to change, and may contain errors. Any errors in this report will be corrected when the final report has been completed.

On May 26, at approximately 1340 Atlantic standard time, a Modeltech ME-109 nonregistered homebuilt airplane, was damaged beyond repair following impact with terrain in St. Thomas, VI. The private pilot was not injured. The owner/pilot was operating the airplane under Title 14 CFR Part 103. Visual meteorological conditions with winds in excess of 10 knots prevailed for the personal flight that originated from Simón Pierre Field approximately 10 seconds before the accident. No flight plan had been filed.

Immediately after takeoff, the pilot realized that his ailerons were not functioning. The pilot declared mayday to Simón Pierre TWR and attempted to perform a short-field landing. The rudder was insufficient to counteract the torque of the engine; the plane entered a steeply banked descending turn which terminated upon impact with the ground. Damage was significant - the fuselage was broken in several places, the firewall was broken off, cowling smashed, and the engine had its valve cover torn off.

Addendum 1 : Upon testing, it was revealed that the receiver's aileron channel was dead. The aileron servo functioned normally when plugged into other channels.

Addendum 2 : The receiver was velcroed directly to the fuselage. No vibration-absorbing material was in use.

The St. Thomas R/C Association Safety Board determines the probable cause(s) of this accident as follows:
CORRECT PREFLIGHT PROCEDURE.. NOT PERFORMED

Contributing Factors
INCORRECT CONFIGURATION OF ONBOARD RADIO GEAR, LEADING TO FATIGUE AND FAILURE OF THE RECEIVER

No shortage of fuel. All the lines were still sealed when I recovered the wreckage - so the spilled-fuel environmental DISASTER was avoided.
