I have personally signed off on 6 waivers in the last couple of years...personally, the flight requirements are now so bone-simple that anyone who is ready to step up should be able to ace them. Frankly, I spend more time observing the attitude of the 'applicant', as well as his/her ability to think ahead of the plane and put it where they want it. Nothing shows this more than exhibiting good power management, along with the ability to execute a safe, stabilized approach and smooth landing and well controlled rollout.
I've never declined an applicant during the formal demo flight, but at some personal risk (beer story, not for here), I did tell one prospective applicant that he was not ready and to go practice some more before doing the waiver flight. He was pissed at me, but later thanked me - and DID get his waiver (with me as one witness) after getting some help and succesful buddy-box instruction a few months later.
I'd rather have a PO'ed club member, than have a major incident (fire, etc) with my name attached to it. May not be PC, but oh well... much better for the hobby in the long run.
