it most likely wouldn't run, or stay running. The best thing to do would be to flush the engine with plenty of fuel or after run oil, and manually turn the crank a few times. If a very expensive engine, or you are on a budget, then taking apart the entire block and cleaning is a better idea, as the above can still cause damage. This is a very time consuming process though.

If the piston or sleeve were scratched from the sand would it only run for a short period of time? then the sleeve will expand and kill the engine?

That could be a symptom as well, yes.

It depends on how bad things are scratched up. If you think something's been sucked in, I would immediately pull it from your ST and tear it down to clean it. When you do, inspect the piston and sleeve for scratches, and you'll have your answer to how bad it's damaged.

Well there could be alot of symptoms, not idleing, dies when it warms up, poor performance, hard to start. Those are just off the top of my head.
It depends how bad it is also, if you barley got any in there, and it didn't even pass the carb. You may not see any symptoms, at first.
