A loose muffler cover like your description is a problem, but it RARELY causes an engine to fail.

Yes. Check your pulleys at the tailbox and the secondary 'main' gear. They have been known to slip on the shaft over time and aggressive flying.

Up and down is usually a blade imbalance.
Side to side could be gain, low headspeed, too tight/loose...
