Well you build the nacelle, the would go somewhere in the middle, and for the cones, somehow scratch build the cones and make them so that the air flows around them and into the turbine intake. This is just a guess, as I like to draw what I think about on paper.
You can make the two outer nacelles dummys, as you will only then need two engines.
Once again I am just throwing out an idea.
