I don't like the crossed setup as the lines get slightly tighter and looser as the system is rotated. Some like them crossed because you can get a slightly shorter exit slot.

your first picture is right. The servo arm and the pushrod must be a 90 degrees when the surface is at neutral.

Yes, a 5-cell NiMH (6 volt) pack will work fine. But make sure it is of large enough capacity to handle the loads of the servos. Don't put a 800 mAh NiMh pack in a plane with 6 or 8 large servos.
