This sounds a lot like the upper hub/inner shaft is not secured well at the bottom where it mates with its main drive gear. The shaft has two flat spots that the set screws in the collar must mesh...

Sounds like maybe when you attached the main gear to the bottom of the inner shaft, you didn't get the set screws mated with the flat spots on the shaft (on all the ones I've seen, they are aligned...
