is it ok if the exhaust exit on the stern is under water

When the boat is moving on the water, unless the boat is going backwards, the exhaust coming through the stern (Transom) will not be under water. Water flowing under the boat follows the bottom line until it's well behind the boat. The only issue would be when the boat stops. Then water can run into the exhaust and if you lift the boat out of the water stern first it will run into the engine. OK if you run it again but not OK if you take it home and leave the water in the engine. Rusty bearings and crankshaft will be your reward.
