What about #5 - miniature spark plugs were made that way (perhaps before unified threads?) and glow plugs evolved in a process that saw them fitted to existing thread cavities.

Good advice, I totally agree - but in 99% of the cases we are talking about cleaning up existing threads, versus cutting a new tap hole - so I have to think that a lower quality tap that meets spec...
