Invariably these kind of incidents occur when an individual departs from a preset and practiced routine. I work in safety in very high hazard environments, and human error is far more likely when the proper routine/procedure is not followed. We must be absolutely dedicated to following the same routine each and every time. As soon as you recognize you have departed from your routine, START OVER from the beginning.

For me, its

1. Place the plane in the restraint.
2. Fuel the plane.
3. Stand on the left side behind the main wing.
4. verify all switches are off.
5. Turn on the transmitter.
6. Turn on the reciever.
7. Verify control surface movement.
8. Turn on ignition.
9. Verify ignition kill works (red led).
10. Verify plane is in the tailhook.
11. Move to the front of the plane.
12. Grab the spinner firmly and set the choke.
13. Flip until the engine barks,
14. Turn off Choke.
15. Start the engine.
16. Move away from the prop and get behind the plane.
17. After the flight, all switches off, the plane goes back in the holder.
Same routine, every time.

Its good to have someone else at the field if the worst happens, but its far better to prevent it in the first place.
