Default DX7 fail-safe

I put the plane out and taxied out and the plane suddenly went full throttle to the other side of the runway and flipped over. Plane went into fail-safe and stupid me forgot to re-Bind after reversing the throttle. Couple weeks later, after flying it for a few times with the fail-safe set, I lost total control of the plane - luckly I was just taxing the plane. Again, the plane went into fail safe mode. Thinking it was a bad receiver I did not fly the plane for a couple weeks. During that time I have been flying other planes with no glitch. Today, I decided to fly my helicopter after a few month in storage and after testing everything and bringing the heli to the pad I lost control of the helicopter. It went into fail safe mode. I pulled out a plane I was flying earlier that morning thinking - could it be my radio. turned on the tx and the rx on the plane and nothing abnormal happened. repeated that a couple times and on the 5th or 7th time the plane went into fail safe mode on the bench. I tried it on the helicopter and again after a few on / off tries it happened again on the heli. the fail safe mode doesn't happen suddenly - I have full control for a few minutes and then it happens. I checked the Transmitter LCD and it is on. no power loss on the LCD. Later that evening I opened the TX case and the antenna is securely connected. don't see any chaffing of the wires inside.

Could my DX7 be going bad?

I have a Lipo TX battery - fully charged
