Default Beginner needs some advice

I am having some trouble with servo movement. I have my flysky fs-th9x transmitter and receiver bound. I have it setup for a 120 degree swashplate, heli 3-1, mode 2, ppm. The problem I am having is the pitch servo only moves when the throttle stick is moved up or down, moves the same direction as the aileron and elevator servo. Doesn't move at all when moving aileron right to left or elevator up and down. On the display channel it shows the pitch deflecting normally but the servo isn't. Do I have a problem with mixing channels? I tried reprogramming the controller to default settings, still having the same problem. The channels are setup as follows ch1 aileron ch2 elevator ch3 throttle ch4 rudder ch5 pitch. If there is anyone familiar with this radio, I'd like some advice. Also, not getting any blade movement when moving throttle stick. Have a straight linear line in throttle curve setup. When I turn throttle hold switch off, blades rotate half revolution and then stop.
