Here's a couple samples of all that you need for your documentation. Line drawings (3-views) are NOT required for any of the classes. If you want, you can include a 3-view on the backside of your photo. But it's not necessary.
I'm not familiar with the squadron colors of your model as you describe it, but then I'm not an expert on the P-47. The artist rendering is as close as I can find.
