Default ServoCity Motor Controllers Blowing...why?

I’ve sent in an RMA for my 3 motor controllers, but I’m really trying to figure out exactly why they are blowing out the reverse voltage. I was under the impression when I ordered these ServoCity Controllers that they had “bullet proof” protection. I have assumed that the controllers had diode protection, but the only thing I can think that may be occurring is that back fed voltage from the motors are affecting the controllers. Forward (+ positive) voltage is fine on all of the controllers. I’ve attached a drawing of the design in order to illustrate how everything is wired up. Please let me know if you see anything that I’ve missed. I’ve requested an RMA as these were hooked up according to direction, but they blew with motors rated for use with them.

This is a 1:1They all bench tested fine. It’s like it was working fine, when initially connected, and as I was “tweaking” the remote control aspects, things went bad.

I've attached a drawing of the design. Please let me know if you see something I missed. Any help is appreciated.
