Don, from what I remember it was packaged by Airtronics when they manufactured sailplanes. Last time I needed some I bought a roll of the super thin 3M double sided tape in 1/2" width and applied it to some 3 mil Mylar to make my own. It was nice to be able to make the width whatever I needed.
