It could possibly be a bolt tightness issue - did you ever check the cylinder bolts?
It could have been a bolt issue, who knows, anyway I have ordered a new cylinder head and new piston ring. When I replace this I am going to thread lock all the cylider bolts and all the back plate bolts. I'm also just glad this failure happend on the ground rather than in flight as it could have been much worse. I have to wait 3 weeks for the new parts when they come I'll post some pics of the repair.
