The second thing I'd do is make sure you are plugging in the balancer leads on the pack and using the charger's LiPo setting for "Balance Function".

If you are using the wrong chemistry, not plugging in the pack's balancer lead or not using a balance charge option, then it is typical (and very bad) that one cell will over-charge and at some point fail. That failure may include bursting into flames while it is being charged.
