If you are using the wrong chemistry, not plugging in the pack's balancer lead or not using a balance charge option, then it is typical (and very bad) that one cell will over-charge and at some point fail. That failure may include bursting into flames while it is being charged.

Thanks for your prompt reply with helpful information and the link to the detailed manual. I will now always use the balance charge setting when charging LiPo's .
