Sorry, nylon bolt into wood. I assume the wood was reinforced with CA but I didn't build it. It seemed snug enough but apparently not.
I would add CA and re-tap the threads,,, do it a few times,,
I wonder if someone used metal bolts at one time and vibration enlarged the threads,, very odd that a nylon bolt would work loose out of good threads
