It's a personal preference thing.
I fly pinch, though I've gotten lazy from all the flight instruction and combat flying, so I find myself using pinch on the right stick and thumb-only on the left stick (so I can hold the trainer switch, etc). I also have short-stubby fingers, on many transmitters I have a hard time getting the thumb all the way to the "inside" (I roll right most of the time, guess why). So I use a neckstrap. This allows me to lift my hands up on the TX more so I get better reach with my short fingers and I feel I have better control.
pkh, I also do a TON of hand launching, since I fly combat. Try learning the left-hand underhanded toss. Unless you're chucking some underpowered planes, once you get used to it, it's a lot easier, and you start with your right hand on the sticks. I can start my plane on it's start-stand, put the TX and strap on, pick up the plane with my left hand, throttle up with my mouth or chin, then toss the plane, all quickly and easily in the 90 second window. I think it's faster and safer than trying to get under the plane and throwing overhand or righty. I also put a bit of stair tread on my planes for a good grip. I can launch an SSC plane in a 15mph tail wind with no problems with this technique.
