Timing Question 6 vs 12...does it matter?

Does it matter if you insert the cam gear in at 6 or 12 o'clock when the piston is at tdc? Reason I ask is because if you rotate the crank 360 degrees and had installed the gear at 6 o'clock, you end up with the piston at tdc and the cam gear at 12. If it does matter then, at which part of the 4 cycle do you install at 12 o'clock?
FYI I run a YS 1.20 and had problems with the first start of the season. I had cleaned the engine and reassembled it. It started up but wouldn't do much past idle. It actually sagged a lot then quit when I gave it some throttle. I had installed the cam gear at 6 o'clock thinking there was no difference given my question above. When I tried to start it again, it wouldn't even kick. After it cooled down, I tried it again and it started up but with the same problems. Tried to start again...no dice. Let it cool down and it started, but, with the same problem.
