Surprisingly, it doesn't matter if the dimple on the cam is 12 O-Clock or 6 O-Clock just so it is one or the other when the piston is all the way up. The cam only travels half the speed of the crankshaft. One time the piston is at the top the dimple will be at 6 and the next time it will be a 12.
