There is a scan of the template in an earlier thread about the 40size Ultimate.

If you open the attached photo and copy it down to your pc, you will have a picture that's easy to use to make your own.

I scanned the template with a ruler beside it in case your printout software screws up the dimensions. I did test to see if a printout of the attachment comes out dimensionally correct on my pc/printer and it did. That doesn't mean it'll come out correct for everyone. If it doesn't, look at the picture with a paint program that will give dimensions and ppi values. That'll give you a clue how to correct for your printer etc.
