And, is the shaft supposed to come through the top or bottom and does it matter? I do not see any other collars or screws. I tried the blow dryer and it will barely budge.

the shaft and the gear have a D Shape to them. you might have to tap it lightly in the center to get it out. Have someone hold it for you to help.
