Jim,
Just add the ceramic blanket per the template. Cover the ceramic material with aluminum foil using high temp RTV as an adhesive. Then attach the blanket to the fuselage with the same RTV.
I have painted the last 3 or 4 inches of the bypass tube with BVM Heat Shield on the inside of the bypass tube as shown on page 48 of the assembly manual. I did not find iot necessary tio add Heat Shield to any other part of the aft fuselage.
