I found an interesting report on the homepage of the norwegian accident investigation commission. A fullsize pilot who reversed his ailerons!

so that was what that sound I heard was.... many mechanics and inspectors simutaniously slapping their foreheads and saying DOH!

Recently an experienced pilot took off his beautiful large scale Extra at our field only to realize shortly after liftoff that he had forgotten to hook up the ailerons when he assembled the plane. He flew it around for about 20 minutes on rudder and elevator making several unsuccessful attempts to land. The flight finally ended in the destruction of the plane. So yes, do do your prelight checks!
