Default filing a faa 333 exemption...

Doing some research on the web and stuck in a circle of links trying to figure out what an application for exemption for the faa actually looks like. I thought it would be a form to fill out, but it looks like you have to compose an actual letter for review by a committee. Anyone successfully do this? I work for a grain elevator and we would like to use a dji phantom to survey crops for our farmers, and also take aerial photos of our facility for our website. A quick search on the net will find a bunch of law firms willing to file for you for $1500-4000, but that seems unrealistic since the regulations are constantly changing and who knows if you'll even need this in a year, or if you'll need something else instead... help!
