What can cause an electric motor to reverse direction on its' own?

Highly unlikely once it is installed. If you are running a car ESC on and airplane you could accidentally engage reverse which would spin it the other way.

I suppose an airplane ESC could be out of sync when it starts but that is unlikely.

Direction of the motor will depend on how you hook up the power leads from the ESC. 2 leads for a brushed motor and 3 for a brushless.

Reverse the leads on a brushed the motor will spin the other way.

Switch any two leads on a brushless and it will spin the other way.

So, the question becomes, what did YOU do to cause the motor to spin the other way?
