Default XTM X-Terminator clutch problems

The clutch bell keeps spinning off the washers and the stud that holds it on.
I have bought and used like $15 clutch shim kits and have used all of the shimms..they just keep spinning the washers off.
Any suggestions?
Would an improper engine alignment cause this problem?

Oh yeah..

Stock setup has 2 bearings.. Have burnt them out like 2 times..

The third time i put 3 bearings in the clutch bell.
