On your way down, you need to stop at the Canada customs office (not the US office) to get one. I'm not aware of any other places to get one.
Some refer to the Y38 as a 'green card' due to the colour. Its about the size of 2 credit cards. The card has a general description of critical items, and documents serial numbers for each. If the item doesn't have a serial number (ex/ radio control jet...) the customs officer will apply a non-removable code sticker and record that code in place of a serial number.
From experience, they don't usually ask for it when reentering Canada, unless the jet looks really new or like it may have just been purchased. I wouldn't worry about having them document every servo, battery, etc, just the big stuff. More often that not, when the customs agent sees that I have a stack of green cards, they don't bother asking for them. If you don't have documentation though, it opens up the opportunity for a difficult examination of everything to make sure you are paying your taxes...
Only other tip, when you go to get the green card(s), bring along whatever receipts you have. I've been asked to prove that an item was imported properly in the past - before they would give me the card.
