The only problem you will have is similar to a radial. You will be fine but expect smoke when you first start up because every bit of fuel and oil will be in the low cylinder. You might have a plug fowling problem. Just be sure to run the engine dry until it quits after every flight or at least after every day of flying. This might solve the smoke or fouling problem al together.

Would the top cylinder run leaner than the bottom?

I'm wondering about another thing. The air flow will be very different in the top and bottom of your cowl. Will the top cylinder run hotter? I think it may. Enough to cause concern, not sure. Baffles would be in order at any rate.

gravity!!!!! the fuel will naturally fall to the bottom cylinder. you'll have to run richer to keep the top cylinder fueled which will cause the bottom to be in a rich condition all of the time.

For all the above, I don't think I would go this route. The very small blisters Wayne D put in his cowl to cover the plug caps really don't look bad. In say, a checkerboard scheme, I don't think they would be that noticeable.
