I would venture to offer that it was not pilot error. There is entirely a possiblity the pitch trim waa backwards. The airplane is nosing up unexpectedly, so he feeds in down trim to no help. There obviously no real prep for a first test flight. You don't load a pax in the back on a brand new test flight. So likewise I'll bet it was a very incomplete post maintenance preflight.
