If you read the Florida Jet 2005 thread you'll see that a) it is a Cessna Skyvan and not a bright red and black aerobatic model of the Raven, b) the cause was reversed ailerons and c) he was one of the most experienced pilots there and was simply distracted throughout his pre-flights.

Goes to show that wiggling the stick and seeing the surfaces wiggle too isn't always enough. Moral of the story - don't get distracted during your pre-flights!
