you've tried setting the idle so low it almost stalls?

If you have, then it is most likely a problem wtih the clutch staying engaged.
like fantomas says, broken clutch spring, or possible bearing failure in the Clutch bell.

But basically, the only way for it to stay moving is for that clutch to be engaging at least partially...
