Biggest problem I've seen is a construction manual obviously written by someone who never actually put a plane together - specificially a Bobcat 50. Wrong screws were specified for several assembly steps, and assembly called for glueing the wings on before assembling the wing pylons. Biggest problem however, was that the fuel tank was shown installed with the plug end facing the pusher engine. In this configuration the plane would stall the minute the plane pulled up.
