This is one maintenance issue many people overlook, and it can be a costly one! Nylon insert hex nuts dont last forever! Regular replacement is required, and should probably be done about once a month or so (depending on use, and how often the wheels are taken off) If you can screw on the nut by hand, then the nylon insert is all but useless. If you dont want to replace nuts, then use loctite.
