You are checking off the balance connector, which is fine, but your number for each cell are way off. If you measure the voltage from the JST connector, it should read right at 12.4 to 12.5 volts. On the balancing connector keeping the negative voltmeter lead on the black balancing plug pin, you should be able to reead across the connector by checking each pin. 1rst pin will read one cell..... next pin will read 2 cells........last pin will read all 3 cells.......

Your reading showed 3 volts on one cell...and 4 in the other 2....that means your pack is way out of balance. Take a reading off the output (JST) connector and see what you have.
