Default Li-Po Problem

I have a Polyquest 800 mah 3 cell lithium that I am having problems with. I charge it up and my charger says that it is done, but I read it on a voltmeter and it is only reading 11.07 volts. I just had this problem this morning when I took off and noticed I didn't have power 30 seconds into the flight. I don't know what could be causing my problems. If anyone has any suggestions, they are greatly appreciated.

11 volts indicates empty. Full charge should be 12.6 volts. Maybe a charger problem.
