Check to make sure that all the wheels are in alignment (see piccy below). You may need to tighten the suspension arm screws as these can swing away from the tank and cause the track to come off (red arrow in the piccy below.
Also do you have metal tracks?
I had this problem when I fitted metal tracks and I only sorted the problem out when I fitted metal suspension arms (these don't flex like the plastic ones.)
The track would try and pop off every time I turned or went into reverse but since fitting the metal arms I've driven for over 3 hours and the tracks have behaved themselves.
